Are you looking for a role where no 2 days are the same?
We are looking for motivated and compassionate Band 5 Community Nurses to join our dynamic locality teams. This is an exciting opportunity to deliver high quality person centered nursing care to people in their own homes, supporting them to remain independent, safe and well within their community.
This role is ideal for newly qualified nurses, those seeking their first community post, or nurses wanting to broaden their skills and progress with locality teams, within a well structured career pathway.
You will be offered a structured preceptorship and as a community nurse working within the planned care service , you will be providing high quality care for people in their own homes, ranging from long term conditions, to complex and palliative care. You’ll be working on a rotational basis as part of a seven-day service, with the support of agile technology to help you deliver timely, high quality care, with support from specialists across the Trust.
Shift patterns are between the hours of 8am and 9pm, 7 days per week and We have vacancies across multiple community locality teams.

To work as part of the district nursing team to ensure high quality health care is delivered to patients in their own home and/ or clinic setting. To work in collaboration with the wider Primary Health Care team, whilst participating in the assessment of health needs and delivery of care to the practice population. The service is delivered over a 7 day rota and postholders are expected to work flexibly in line with agreed operating hours to cover a 24 hour service
