Your Role:
We have an exciting vacancy for an experienced nurse to provide specialist, high‑acuity care to patients with complex needs in their own homes.
This opportunity is ideal for a confident, autonomous nurse who thrives in a fast‑paced environment and is comfortable managing complex and sometimes challenging clinical situations. Given the responsive nature of this role, you will need a very flexible approach to the rota, including the ability to work some unsociable hours or regular overnight stays, and to support short‑notice patient visits where required to maintain service continuity.
Key Details:
Location: Working with trusts and patients in the South, with occasionally the need to work with trusts and patients nationally.
Working hours: Variable hours across Monday - Sunday to meet rapid response demand. Flexibility is essential; shifts may be arranged at short notice depending on patient need.
Salary: £51,703
Your main responsibilities:
Respond rapidly to urgent clinical requests, providing timely assessment and intervention to support patients in the community.
Deliver high‑quality care including IV therapy management, clinical observations, medication administration and escalation where required.
Lead safe, effective and rapid discharge pathways by working alongside BDMs and RNMs
Work autonomously to prioritise and manage your workload based on clinical urgency.
Communicate effectively with patients, carers, and wider multidisciplinary teams to ensure continuity of care.
Document care accurately and promptly in accordance with company and NMC standards.
What you need to succeed:
Registered Nurse (Adult) with current NMC registration.
Proven experience working as a field‑based nurse, demonstrating your ability to manage your own schedule, work autonomously, and deliver high‑quality care in patients’ homes or community environments.
Extensive knowledge and experience of Home Parenteral Nutrition (HPN), including CVAD management.
Competence in the safe administration of intravenous drugs and infusions, with a strong commitment to clinical safety and adherence to protocols.
Strong conflict‑management skills, enabling you to handle challenging situations calmly and professionally while maintaining positive patient and family relationships.
A proactive and adaptable approach, with the ability to make sound clinical decisions in time‑sensitive circumstances.
Excellent communication and documentation skills, ensuring accurate, timely and compliant records.
Willingness and ability to travel extensively, including staying away from home regularly, to provide responsive support to patients across the country.
Full UK driving licence.
