We’re seeking a Registered or Enrolled Nurse to join our Charlestown Medical Centre, where you’ll support GPs and patients through chronic condition management and preventative healthcare. You’ll be part of a great, supportive team that values collaboration, culture, and patient outcomes. Part-time and full-time opportunities available.

What You’ll Do:

Prepare Care Plans and Team Care Arrangements

Support patients in managing chronic conditions and improving health behaviours

Conduct health assessments and promote preventative care

Collaborate with GPs and multidisciplinary teams

Manage administrative tasks related to CCM appointments

About You:

AHPRA registered Enrolled or Registered Nurse

Prior experience in Chronic Condition Management preferred

Strong communication and organisational skills

Friendly, proactive, and patient-focused approach
