This role offers the opportunity to make a meaningful difference at the front door of the health system, supporting people to access the right care at the right time. As a Registered Nurse in the Triple I Hub, you will provide safe, compassionate, and person centred nursing care through clinical assessment, advice, and coordination. Working closely with patients, families, and multidisciplinary teams, you will support timely decision making, appropriate escalation, and referral to services across acute, community, and primary care settings.
This role plays a critical part in supporting patient flow, admission avoidance, and continuity of care, helping reduce unnecessary emergency department presentations and delays to accessing care. You will contribute to a responsive, centrally coordinated service that improves patient experience while supporting broader system sustainability.Within the team, your role matters because it strengthens service capacity, supports colleagues during peak demand, and helps maintain safe, high quality care. More broadly, the role contributes to healthier communities by ensuring people receive timely guidance and coordinated support when they need it most.

You are a Registered Nurse with strong clinical judgement and a genuine commitment to supporting people to access the right care at the right time.
You are confident in assessing patient needs, thinking critically, and making timely decisions that support safe escalation, referral, and care coordination. This is an office‑based role within the Triple I Hub. It does not involve home visits or community nursing. Instead, you will support patients, families, and carers, over the phone, providing clinical advice, and care navigation in collaboration with multidisciplinary teams and service partners across the health system.
You communicate clearly and empathetically, are comfortable managing conversations with people who may be distressed or uncertain, and can explain options in a calm and supportive way. You work well in a fast‑paced environment, manage competing priorities, and remain focused on patient safety and experience.
You are organised, adaptable, and accountable for your practice, with an interest in contributing to service quality, patient flow, and system efficiency, particularly during periods of increased demand. If you enjoy clinical problem‑solving, collaboration, and making an impact without face‑to‑face visits, this role will suit you well.
Selection Criteria

Current Authority to Practice as a Registered Nurse with Australian Health Practitioners Regulation Agency (AHPRA).
Demonstrated recent experience as a Registered Nurse, with the ability to assess, plan, implement, and evaluate safe, compassionate, and evidence based nursing care appropriate to the clinical context.
Demonstrated ability to communicate effectively and empathetically with patients, families, and carers from diverse backgrounds, using a person centred approach to identify needs, support decision making, and ensure culturally responsive care.
Demonstrated capacity to use critical thinking to manage competing priorities, recognise risk, escalate concerns appropriately, and take responsibility and accountability for clinical practice and professional standards.
Demonstrated ability to work collaboratively within a multidisciplinary team and with external services, while contributing positively to team culture, risk management, work health and safety, and continuous quality improvement.
Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
