Access Aged Care is scaling fast across the Central Coast. We need nurses who can step in, take ownership, and keep clinical care moving.
This is not a passive role. You will be on site, making decisions, coordinating care, and ensuring residents don’t fall through the gaps. If you prefer routine bedside task based nursing, this is not the role for you. If you want to use your clinical judgement every day, it is.

What You’ll Do
Work on site as a 3rd party in RACHs as the Access Aged Care nurse
Facilitate and coordinate residential care alongside the facility nurses in the home
Run GP and specialist rounds, in person and via telehealth
Identify issues early and act before they escalate
Drive referrals, follow ups and clinical coordination
Complete assessments
Be the link between care teams, GPs and families
Build strong relationships and hold standards of care
Use a wide range of software systems and clinical judgement to improve outcomes

What You’ll Bring
Diploma or Bachelor of Nursing with AHPRA registration with 2 years experience post graduation
Aged care experience or similar clinical background
Strong judgement and ability to prioritise under pressure
Clear, confident communication with multiple stakeholders
Comfort with technology and reliable with documentation
Driver’s licence and reliable vehicle
