About the job
To aid in the effective coordination of discharges across all adult inpatient areas and the emergency department while ensuring a positive patient experience.
Discharge planning including coordination and management of complex discharges involving patients with multiple comorbidities, ongoing care needs, or social care requirements
Responsible for providing safe, high-quality care to patients undergoing planned procedures across a diverse range of specialties
Maintain standards in accordance with ward /Trust policies and procedures and NMC code of conduct
Maintain a safe working environment, being aware of the necessity to comply with Health & Safety regulations and the Environmental Health Act, and to attend mandatory training courses.
Admitting elective patients
Escorting patients to theatre
Recovering day case patients and escalating any concerns to the relevant teams
Co-ordinating discharges
Liaising with external agencies to ensure safe discharge from hospital
Completing relevant referrals
Providing nurse led education
Comprehensive discharge planning and problem solving
